The State of Illinois requires the following exams:
Physical Exam: an exam is required for students entering Preschool, Kindergarten, Sixth Grade or entering MSW for the first time within 30 days of enrollment.
Dental Exam: an exam is required for students in Kindergarten, Second and Sixth Grades.
Vision Exam: an exam by an optometrist or physician who provides complete eye care is required for students entering Kindergarten or entering MSW for the first time within 30 days of enrollment.
