Please complete the registration forms and submit the following information to complete your application for enrollment:

1. A copy of the child’s birth certificate from the Cook County Office of Vital Records.  You can get quicker service if you place your request on your credit card.  Your Certificate will leave their office within 24 hours.  Each additional copy is $2.00.  Their phone number is 312-603-7790.
2. A copy of your child’s baptismal certificate.  Call the church of baptism, and request a copy to be mailed to you.  If baptized at MSW, you do not need the certificate, but you do need the month, day and year of baptism.
3. NON-REFUNDABLE registration fee of $100.00.
4. If you are a registered, contributing member of Mary, Seat of Wisdom be sure you have your parish number with you.  It can be found on your Sunday church donation envelope.  Families applying for grades K-8 will not be eligible for parishioner tuition rates until we have a parishioner number.
5. The following age requirements apply:
   a. Students entering the 3-year old preschool program must be 3 years of age by September 1.
   b. Students entering the 4-year old preschool program must be 4 years of age by September 1.
   c. Students entering Kindergarten must be 5 years of age by September 1. 
6. For each student registering at MSW for the first time in grades 1 through 8, a copy of the student’s last report card along with a completed request form for transfer of permanent records must be presented in addition to the information listed above.
7. For each student registering at MSW for the first time, a medical and eye exam are required.  For new students entering Kindergarten, 2nd or 6th grade, a dental exam is also required.  These forms are due in the office by July 1st. 
8. A medical exam is required for all students in Grades 4-8 who participate in athletics.
9. Students who require medication during the school day must complete the medical authorization form. 

As we are not always able to accept all registration applications we receive due to class size, we encourage you to complete all required forms and provide all documents as quickly as possible.  Completion of the registration form does not ensure acceptance into the grade or preschool program.  Registration forms will be collected in the school office until March 1.  At that time, the criteria for admissions as established by the School Board will be applied.  This criteria includes acceptance of the following (in this order):

1.  Children already accepted and attending MSW and their brothers and sisters.

2.  Catholic children of registered parishioners.

3.  Catholic children of non-parishioners.

4.  Non-catholic children.

(If the cut off point lies within a criteria group, a lottery will be used to determine acceptance among those applications.)
Parents will be notified by April 1 of the acceptance status of the registration.  Additional registrations will be accepted after March 1, if space permits.
