2016-17 Back-to-School Information
Our Mary, Seat of Wisdom Back to School picnic will be held on Wednesday, August 10, 2016 from 5:00-8:00 PM.
Some of the required paper forms have been eliminated this year due to the addition of our new InfoSnap registration tool, however, you are still required to submit the forms below. Bring these paper forms to the Back-to-School Picnic on August 10th and you will receive your child's homeroom assignment. Room assignments will not be available before that date.
If you cannot attend the picnic, please bring the forms to the School Office after August 10th to receive your child's homeroom assignment. Forms can be mailed to the School Office ahead of time, however room assignments will still not be available until the night of the picnic.

BACK TO SCHOOL FORMS
MILK ORDER FORM - ALL families: Return this form whether or not you are participating in the Milk Program.
FAMILY DIRECTORY FORM - All student names will appear in the 2016-17 Family Directory booklet. Return this form only if your contact information in last year's booklet has changed. FORMS DUE SEPTEMBER 2, 2016.
EXTENDED DAY PROGRAM (EDP) REGISTRATION - Return this form if you will be registering for EDP.
ALL Illinois State Board of Education health requirements must be completed by the first day of attendance in order for your child to begin school. http://www.isbe.net/research/htmls/immunization.htm
PHYSICAL EXAMINATION FORM - Students entering Pre-K, Kindergarten, 6th Grade and ALL students enrolling for the first time. http://www.isbe.net/research/pdfs/immunization-guidance.pdf

MEDICATION PROCEDURES AND AUTHORIZATION FORM - If you chose "YES" in InfoSnap to authorize MSW to administer ANY medication, you are required to provide this Medication Authorization Form.
FOOD ALLERGY & ANAPHYLAXIS EMERGENCY FORM - Return this form if applicable.
SCHOOL SUPPLY LISTS - Online ordering is no longer available. Please refer to these lists when shopping for school supplies.

LUNCH PROGRAM provided by Marla's Brown Bag Lunches. Online ordering begins Monday August 15th. Order by the week, month or entire year. Use the weekly lockout on Tuesday at 9am to change, cancel, or edit your orders weekly. If you participated in the Hot Lunch Program last year, you will still need to create a NEW username this year.

REST MATS - Rest mats for grades PreK and K will be available for purchase at the picnic for $28 each. Call the school office if interested in purchasing a rest mat.

SPIRITWEAR - MSW Spiritwear will be available for purchase throughout the year. A representative will be at the back-to-school picnic.
Spiritwear days are as follows (subject to change): September 9, October 7, November 18, December 9, January 13, February 10, March 10, April 7, May 12. Note: Spiritwear is optional and not required uniform apparel. Spiritwear Order Form 8-4-16
CATHOLIC IDENTITY SHIRT - These shirts are to be worn on assembly days and other designated school days. This shirt is free for all students.
