Welcome Back to School Picnic Forms & Information
Please bring these forms to the Welcome Back-to-School Picnic on August 17th. Once all necessary forms are turned in on that night, you will receive your child's room assignment. Room assignments are not available before that date. If you will be on vacation on August 17th, please bring the forms to the School Office upon your return to receive your child's room assignment. Forms can be mailed to the School Office ahead of time if you prefer; room assignments will still not be available until after the 17th.
We have indicated after each form whether we need one per student or one per family.

Archdiocese of Chicago Medical Procedures Information (please read; does not need to be turned in)
Archdiocese of Chicago Medication Authorization (one for each student who needs medication)
Photo / Academic Work Permission Form (one per student)
Acceptable Use of Technology (one per family)
Milk Order Form (one per family - even if you are not ordering milk)
